Before you start: the pain rule

A helpful guideline is to work into mild discomfort - a gentle stretch sensation that eases soon after - but to stop short of sharp or lasting pain. If an exercise leaves your shoulder more painful for hours afterward, it was too much. Warming the shoulder first (a warm shower or heat pack) can make movement easier. If you are in the early, very painful "freezing" stage, keep everything especially gentle and check with a physiotherapist. Always confirm a program suits your stage with a healthcare professional.

Gentle exercises to try

1. Pendulum swings

Lean forward slightly, supporting yourself with your good arm on a table, and let the affected arm hang relaxed. Gently swing it in small circles and back-and-forth, letting gravity do the work rather than muscling the movement. This is one of the most forgiving early exercises.

2. Assisted forward reach with a stick or towel

Hold a light stick (or rolled towel) with both hands. Use your good arm to gently guide the affected arm forward and upward, only as far as comfortable. The stronger side helps the stiff side move without straining it.

3. Cross-body stretch

Bring the affected arm gently across your chest, using the other hand to support it at the elbow. Hold for a slow count, feeling a mild stretch across the back of the shoulder, then release.

4. Wall walk

Face a wall, place your fingertips on it, and slowly "walk" them upward as far as comfortable, then back down. This gives a gentle, controlled way to explore overhead range.

5. Towel behind the back

Hold a towel behind your back with both hands - one over the shoulder, one at the lower back. Gently use the upper hand to draw the lower arm upward, easing internal rotation, which is often the stiffest direction.

How often?

What to avoid

  • Forcing through sharp pain. Aggressive stretching can inflame the capsule and set you back.
  • Heavy lifting overhead while the shoulder is stiff and irritable.
  • Sudden, jerky or ballistic movements - bouncing into a stretch instead of easing into it.
  • Locking the shoulder up completely. Total immobility can worsen stiffness; gentle motion is generally better than none.
  • High-impact or high-load gym work on the affected side until you have guidance that you are ready.
  • Comparing to your other shoulder day by day. Progress with frozen shoulder is slow and uneven, and that is normal.

Why gentle wins

The tight capsule in frozen shoulder does not respond well to being forced. Think of it like easing open a stiff hinge rather than yanking it. Gentle, repeated movement encourages the shoulder to reclaim range gradually, while respecting the inflammation that is part of the condition. Many people regain good function this way over time.

When exercises are not enough

If you are doing everything consistently and pain or stiffness is still blocking progress, it may be time to revisit your plan.
