What each procedure does
A genioplasty, often called a sliding genioplasty, is a bone operation. Through an incision inside the mouth, the surgeon cuts the lower part of the chin bone, moves it to the planned position, and fixes it with small plates and screws. Because the bone itself moves, the chin can go forward, back, up, down, or to one side, and can be made shorter or longer.
Both can give a stronger, squarer chin. The real question is how much change you need, and in which directions.
What the research says
A 2025 systematic review in Aesthetic Surgery Journal Open Forum compared the two approaches across 7 studies and 1,126 patients, of whom 740 had a genioplasty and 386 an implant. Implants were generally used for mild-to-moderate deficiency and were associated with less discomfort and a faster recovery. Genioplasty was generally preferred for severe deficiency and three-dimensional corrections, such as a chin that is both short and set back. Satisfaction was high with both.
The review also found that the soft tissue of the chin followed the change by about 85% after genioplasty, compared with about 66% after an implant. Your surgeon accounts for this when planning implant size.
Complication figures ranged widely between studies. For implants, infection ranged from 0 to 23.8% and reoperation from 0 to 19%. For genioplasty, infection ranged from 0 to 5.3% and reoperation from 0 to 2.6%. Temporary changes in lip and chin sensation were common after genioplasty, and in the studies that reported it, a lasting sensory change was found in 7.4 to 12.5% of patients. The same review found that persistent numbness was also reported after chin implants, in one older study (9.4% of patients), so a lasting change in lip and chin sensation is possible with either approach.
