The shoulder is a remarkably mobile joint, and that mobility comes with many moving parts that can become irritated, worn, or inflamed. This guide explains the usual suspects and, importantly, the warning signs that mean you should be seen promptly.
What counts as "chronic"?
Pain is generally described as chronic when it persists beyond about three months, or keeps returning. At that point, hoping it simply disappears is less useful than understanding what is driving it, so care can be targeted.
Common causes of chronic shoulder pain
Rotator cuff problems
The rotator cuff (a group of four muscles and tendons that stabilize and lift the arm) can become worn, inflamed, or torn. This often causes pain on the outer upper arm and weakness with lifting or reaching overhead.
Frozen shoulder (adhesive capsulitis)
Here the shoulder capsule becomes inflamed and tight, causing stiffness in every direction along with aching pain. It differs from a rotator cuff tear in that stiffness, not weakness, dominates - a distinction we explain in frozen shoulder vs rotator cuff tear.
Shoulder osteoarthritis
Age-related wear of the joint cartilage can cause aching, stiffness, and sometimes a grinding sensation, often worse with use and over the years.
Tendinopathy and bursitis
The tendons and the small fluid-filled cushions (bursae) around the shoulder can become irritated - sometimes called impingement - causing pain with specific movements, especially overhead.
Calcific tendinitis
Calcium deposits can form within a tendon, sometimes triggering sharp flare-ups of pain.
Posture, overuse, and the neck
Repetitive strain, prolonged desk posture, and problems in the neck can all contribute. Sometimes what feels like shoulder pain is actually referred pain - pain felt in the shoulder but coming from the neck.
Red flags: when to seek attention promptly
Most shoulder pain is caused by the conditions above and is not dangerous. However, certain symptoms warrant prompt medical assessment, because they can point to something that needs urgent care:
- Pain after significant trauma (a fall or accident), especially with obvious deformity or inability to move the arm - this may indicate a fracture or dislocation.
- Sudden severe weakness or the arm giving way.
- Fever, redness, warmth, or swelling over the joint - possible infection.
- Numbness, tingling, or pins-and-needles spreading down the arm.
- Chest tightness, breathlessness, jaw or left-arm pain alongside shoulder discomfort - seek emergency care immediately, as this can signal a heart problem rather than a shoulder problem.
- Unexplained weight loss, night sweats, or a lump, or a history of cancer, with new shoulder pain.
- Pain that is steadily worsening despite rest and simple measures.
If any of these apply, do not wait - arrange to be seen without delay. For chest, breathing, or heart-related symptoms, treat it as an emergency.
