It sits between everyday medication and surgery, and it is often misunderstood. This guide explains what it means, how it is done, and who it may suit - in plain language.

The idea in one sentence

Interventional pain management uses small, precisely placed injections or needle-based treatments to reach the exact source of pain - a joint, a nerve, or a muscle - rather than treating the whole body with oral medicine. Why "image-guided" matters

"Image-guided" simply means the doctor watches a live picture while working. Depending on the procedure, this may be:

  • Fluoroscopy - a moving X-ray that shows bone and needle position in real time.
  • Ultrasound - sound waves that show soft tissue, muscle and blood vessels without radiation.

Guidance helps place the needle where it is intended to go and helps avoid nearby structures. It is one of the reasons many interventional procedures can be done in a treatment room rather than an operating theatre.

Needle-only, same-day discharge

Most interventional pain procedures are needle-based rather than surgical. There are no large incisions and, in general, no overnight stay. Most patients go home the same day and receive simple aftercare advice. Recovery experiences vary from person to person, but many people return to light daily activity soon after.

What these procedures aim to do

It helps to be honest about the goal. Interventional procedures aim to relieve pain and improve function - to help you move, sleep and get through your day more comfortably. Some are also diagnostic, meaning they help pinpoint which structure is causing pain so that further treatment can be targeted. They are options within a broader care plan alongside physical therapy, activity and, where appropriate, medication - not standalone solutions.

Common options offered by our pain medicine team

  • Diagnostic medial branch block - a small injection near the nerves that supply the facet joints (the small joints at the back of the spine) to test whether those joints are the pain source.
  • Medial branch radiofrequency (RF) - uses controlled heat to calm those same nerves when a block has suggested they are involved.
  • Facet joint injection - medicine (which may be steroid, PRP, or prolotherapy depending on the plan) placed into or around the facet joint.
  • Epidural steroid injection - anti-inflammatory medicine placed in the epidural space around the spinal nerves, often for pain radiating into an arm or leg.
  • Selective cervical nerve root injection - a targeted injection near a specific neck nerve root.
  • Trigger point injection - a small injection into a tight, tender knot in a muscle.
  • Stellate ganglion block (SGB) - an injection near a cluster of nerves in the neck, used for certain pain and related symptoms.
  • TAME (trans-arterial micro-embolization) - a minimally invasive procedure for some types of chronic joint and tendon pain.

Who might consider this route

Interventional options are often discussed when:

  • Pain has persisted despite rest, medication and physical therapy.
  • Pain is interfering with sleep, work or movement.
  • You want to understand the source of pain before deciding on bigger steps.
  • Surgery is not wanted, not suitable, or being weighed against alternatives.
