  • Neck and back pain are among the most common reasons people struggle to sleep, work or move comfortably. Most episodes settle with time, movement and simple measures. But when pain lingers, it can be hard to know what to try next. This article explains when image-guided, needle-only options may be worth discussing with our pain medicine team.

First, the reassuring part

Most new back and neck pain improves within a few weeks with gentle movement, good sleep, and activity as tolerated. Rest for very short periods, staying active, and appropriate exercise are usually the foundation. Interventional procedures are not a first step - they are considered when simpler measures have been given a fair chance and pain persists.

Signs it may be time to look further

Consider a pain assessment when:

  • Pain has lasted more than 6-12 weeks despite sensible self-care and physical therapy.
  • Pain radiates into an arm or leg, or comes with pins-and-needles or weakness.
  • Pain is disrupting sleep, work or your ability to exercise.
  • Medication is helping less, or you would prefer to reduce reliance on it.
  • You want to understand the source of your pain before deciding on anything bigger.

Red flags that need prompt medical attention

Some symptoms should not wait for a routine appointment. Seek urgent care if you have new loss of bladder or bowel control, numbness around the groin, significant or worsening leg weakness, unexplained weight loss, fever with back pain, or pain following a serious injury. These are uncommon, but they matter.

Where the pain often comes from

Understanding the likely source helps explain the options:

Facet joints

The facet joints are the small joints at the back of the spine that allow it to bend and twist. When irritated, they can cause localized neck or back pain that is often worse with certain movements.

Nerve roots and the epidural space

When a disc or narrowing irritates a spinal nerve, pain can travel down an arm or leg. This is where injections around the nerve or into the epidural space (the space surrounding the spinal nerves) may be considered.

Muscles

Tight, tender muscle knots - sometimes called trigger points - can cause or add to neck and back pain.

Interventional options our pain medicine team may discuss

All of the following are image-guided and needle-only, with same-day discharge in most cases:

  • Diagnostic medial branch block to test whether facet joints are the source.
  • Medial branch radiofrequency (RF) to calm those nerves when the block suggests they are involved.
  • Facet joint injection (steroid, PRP or prolotherapy depending on the plan).
  • Cervical epidural steroid injection for neck-related nerve pain radiating into the arm.
  • Selective cervical nerve root injection targeting a specific nerve root.
  • Trigger point injection for muscle-related pain.

The aim across all of these is to relieve pain and improve function, and in some cases to help identify the source so treatment can be targeted. They are used alongside rehabilitation, not instead of it.

Two people with "back pain" can have very different sources. Our medical team reviews your history, examines you, and looks at any imaging before recommending anything. Because some procedures are diagnostic, the plan is often built step by step - starting simple and reviewing your response before deciding what, if anything, comes next.

What recovery tends to look like

Most patients go home the same day. Many people return to light activity soon after, with specific aftercare advice for the procedure they had. Recovery and how long relief lasts vary between individuals, which is why follow-up is part of the plan.
