Epidural Steroid Injections for Neck and Back Pain: What to Expect

What is an epidural steroid injection?

The epidural space is the area just outside the covering of the spinal nerves. An epidural steroid injection places anti-inflammatory medicine into this space, close to the irritated nerve. The aim is to reduce inflammation around the nerve and, in turn, relieve pain and improve function - for example, when a disc problem or narrowing is pressing on a nerve and causing pain to radiate into a limb.

When it may be considered

An epidural steroid injection may be discussed when:

• Pain radiates from the neck into an arm, or from the low back into a leg (sometimes called sciatica).
• Nerve-related symptoms such as pins-and-needles accompany the pain.
• Simpler measures - movement, physical therapy, medication - have not given enough relief.
• You and your team want to improve comfort enough to progress with rehabilitation.

For neck-related nerve pain, a cervical epidural steroid injection or a selective cervical nerve root injection (a more targeted injection near one specific nerve root) may be the more suitable choice. Your team will explain which fits your situation.

Before the procedure

Our medical team will review your history, examination and imaging, and explain the plan. You may be asked about medicines you take, including blood thinners, and about allergies. Follow any specific preparation advice you are given.

During the procedure - step by step

1. You lie comfortably, usually face down or with your neck supported.

2. The skin is cleaned and numbed with local anesthetic.

3. Using fluoroscopy (live X-ray) or ultrasound, the doctor guides a fine needle toward the epidural space.

4. A small amount of contrast may be used to confirm correct placement.

5. The anti-inflammatory medicine is delivered.

6. The needle is removed and you rest for a short while.

The procedure itself is usually brief. There are no large incisions.

After the procedure

Most patients go home the same day. You will be given aftercare advice, which often includes taking it easy for the rest of the day. Experiences vary:

• Some people feel tenderness at the injection site for a day or two.
• Relief may not be immediate. Steroid medicine can take several days to take effect, so pain may ease gradually.
• Many people return to light activity soon after, following their team's guidance.

What results to expect - honestly

An epidural steroid injection aims to relieve pain and calm nerve irritation. It is not a cure and its effect varies between individuals - some people gain useful relief that lasts weeks to months, others less. It is often used to create a more comfortable window in which to rehabilitate and stay active. Sometimes a repeat is considered after review; your team will advise based on how you respond.
