  • Genicular artery embolization (GAE) - a minimally invasive way of blocking tiny abnormal blood vessels in an arthritic knee to ease pain - is not for everyone. It is an option for a specific group of people. This article walks through who tends to be a good candidate and who usually is not, so you can have a more informed conversation with a medical team.

The short version

GAE is generally considered for people with chronic knee osteoarthritis pain that has not improved enough with the basics, who are not ready for or prefer to avoid knee replacement for now. It is not a first-line treatment and not a substitute for surgery in advanced cases.

Signs GAE may be worth discussing

You may be a candidate if several of the following describe you:

  • Long-standing knee pain. Your pain has lasted many months and is affecting daily life, sleep, or activity.
  • Basics have been tried. You have genuinely given conservative care a fair go - physiotherapy, exercise, weight management, and medication - without enough relief. (Our overview of managing knee osteoarthritis without surgery explains these steps.)
  • Mild to moderate arthritis, or you wish to delay surgery. Your joint has not reached the point where replacement is clearly the best answer, or you are not ready for that step.
  • You want to stay active. Your goal is to reduce pain enough to move, exercise, and function better.
  • Inflammatory-type pain. Pain that flares with activity and swelling, suggesting an inflamed joint lining, is often the kind GAE targets.

Who is usually not a good fit

GAE is generally not suitable when:

  • The arthritis is very advanced. If the joint is severely worn or badly deformed, a replacement may give a better result - see when knee surgery is needed.
  • The pain is not coming from the knee itself. Pain referred from the hip or lower back needs a different approach.
  • There is active infection in or around the knee.
  • Blood vessels are unsuitable. Some people have circulation problems or vessel anatomy that makes the procedure inadvisable; imaging helps determine this.
  • Certain health conditions apply, such as uncontrolled bleeding disorders, severe kidney impairment affecting contrast dye use, or a significant allergy to contrast. Pregnancy also generally rules it out.

This is not a complete list. Suitability is always judged case by case.

How candidacy is actually assessed

A proper assessment usually includes:

  • Your history: how long the pain has lasted, what you have tried, and how it limits you.
  • Examination of the knee to see where the pain comes from and how the joint moves.
  • Imaging, such as X-rays and often an MRI, to understand the degree of arthritis and the state of the joint lining.
  • A general health review, including kidney function and bleeding risk, because these affect safety.

Setting realistic expectations

Even for good candidates, GAE aims to relieve pain, not to cure arthritis or regrow cartilage. Where it helps, relief may last several years in some cases, typically 1-5 years, though results vary by individual. Some people respond well, some partially, and some may need to consider other options later. Being a "candidate" means the odds are reasonable - not that an outcome is promised.

Do I have to try everything else before GAE?

Generally, GAE is considered after the basics - exercise, weight management, physiotherapy, and medication - have not given enough relief. An assessment confirms whether that box is ticked.

Can I have GAE if I already need a knee replacement?
