What is athlete health screening?

Also called pre-participation screening, it is a check-up designed around physical activity. Alongside the usual health review, it pays particular attention to the heart, lungs, and musculoskeletal system - the parts most tested by exercise. The aim is to find issues early, support safe training, and set a performance baseline. It helps reduce risk; it does not remove all risk of injury or illness.

Screening is not only for elite competitors. It is worth considering if you are:

  • Starting or significantly increasing training.
  • Preparing for an endurance event.
  • Returning to sport after illness, injury, or a long break.
  • An active adult, especially with a family history of heart disease or sudden death at a young age.

If you ever have chest pain, unusual breathlessness, palpitations, dizziness, or fainting during exercise, these should be assessed promptly - that is medical evaluation, not routine screening.

What a screening usually includes

1. History and examination

This is the foundation. The doctor asks about symptoms during exercise, past medical and family history (particularly heart conditions), medications, and previous injuries, then performs a physical examination. A careful history is one of the most valuable parts of the whole screen.

2. Heart checks

Because vigorous exercise places real demands on the heart, cardiac screening is central:

  • ECG (electrocardiogram) - a quick recording of the heart's electrical activity that can reveal rhythm or conduction patterns worth investigating.
  • Depending on findings, your doctor may discuss further tests such as an echocardiogram (an ultrasound of the heart) or an exercise stress test.

The goal is to identify, early, the small number of conditions that can make intense exercise risky.

3. Blood pressure and general fitness measures

Resting blood pressure, heart rate, and body measures give useful context for training.

4. Blood tests

5. Musculoskeletal assessment

A look at joints, flexibility, and any past injuries helps reduce the risk of new or recurring problems and can guide training.

Why the heart focus matters

Serious exercise-related heart events are uncommon, but they are the reason screening exists. Many of the underlying conditions cause few or no symptoms in daily life and only reveal themselves under strain. A screening will not catch every possible issue, but a thoughtful history plus an ECG can flag many of the things worth looking into before you push hard.
