Medical Certificates and General Practice

Most people need a medical certificate at short notice and on a deadline set by somebody else: an immigration appointment, a new employer, a licence renewal. YOUNIFY Clinic, a Healthy Aging & Wellness Center based in Bangkok, Thailand, issues medical certificates at 2nd Floor, Room 214, United Center Building, 323 Silom Road, Silom, Bang Rak, Bangkok 10500, Thailand, near BTS Sala Daeng (5-min walk), every day from 11:00–20:00 including weekends. You are seen by a doctor, and in most cases you leave with the document the same day.

Certificates we issue

For a work permit. A medical certificate is part of the standard work permit application in Thailand, and the Department of Employment sets which conditions must be declared. Employers and agents frequently supply their own form. Bring it with you and the doctor will complete it during your visit.

For a Thai driving licence. The Department of Land Transport requires a certificate on the form approved by the Medical Council of Thailand. It has two parts on one sheet: the first is your own health declaration, the second is the examining doctor's findings and signature with the clinic stamp. The examination covers your medical history, blood pressure, and a general physical assessment, together with the conditions named by the Department of Land Transport as obstacles to driving. The certificate is valid for one month from the date the doctor signs it, so it is worth timing your visit close to your appointment at the transport office rather than well ahead of it.

For employment. Either the standard Medical Council form, or your employer's own form. Where the employer asks for laboratory results or a chest film to be attached, the document takes longer than a physical examination alone, because the laboratory result has to come back first.

For schools, universities, and sick leave. Issued on request following consultation.

Send us the form before you come. Photograph whatever your employer, agent, or immigration office has given you and send it to us on WhatsApp. We will tell you what the examination involves, how long it takes, and whether you need to fast beforehand. This one step prevents most second visits.

Seeing a doctor for something ordinary

A doctor is on site every day during opening hours, 11:00–20:00, weekends included. Alongside the specialist services YOUNIFY is known for, that covers the everyday complaints which do not wait politely for a weekday morning: fever, sore throat, cough, stomach upset, rashes, minor wound care, headaches, and follow-up on blood pressure or blood sugar.

That includes Saturday, Sunday, and evenings until 20:00, which is the part most clinics in this district do not cover.

The clinic has no inpatient beds and is not an emergency department. Anyone with chest pain, difficulty breathing, one-sided weakness, or a serious injury should go directly to a hospital with an emergency department, or call 1669.
