It is neither a miracle nor something to fear. It is a medical option that may help some women considerably, may not suit others, and is best decided individually with a doctor after a proper assessment.

What MHT actually is

MHT means replacing some of the hormones - mainly estrogen, and usually progesterone (or a progestogen) for women who still have a uterus - that decline around menopause. Progesterone is added to protect the lining of the uterus when estrogen is used. It can be delivered in different ways, such as tablets, skin patches, gels, or, for local symptoms, vaginal preparations. The type, dose, and route are chosen based on your symptoms, your health history, and your preferences.

The potential benefits

For suitable candidates, MHT is generally considered an effective option for certain menopause symptoms. Reported benefits may include:

  • Relief of hot flashes and night sweats, which are among the symptoms most consistently helped
  • Improvement in vaginal dryness and discomfort, particularly with local (vaginal) estrogen
  • Support for bone health, since estrogen influences bone density
  • Better sleep and quality of life for some women, often as a knock-on effect of fewer night sweats

Benefits vary by individual, and the goal is symptom relief and wellbeing rather than turning back the clock.

The risks and why timing matters

Being balanced means being honest about risks. Depending on the type of therapy, the dose, the route, your age, and how many years you are past menopause, MHT has been associated with certain risks, which may include effects on blood clots, stroke, and - with some combined long-term use - breast tissue. The picture is nuanced: for many women starting therapy closer to the time of menopause and under about 60, the balance of benefits and risks is often considered more favorable, whereas starting much later or continuing very long-term shifts that balance. This is exactly why it is not a decision to make from a headline or a friend's experience.

A note on "bioidentical" claims

You may see marketing suggesting that "bioidentical" or "natural" hormones are automatically safer. The evidence here is nuanced rather than clear-cut, and compounded custom preparations are not held to the same standards as regulated products. It is more accurate to say that the safest approach is an individualised one, using well-studied options at an appropriate dose and route, reviewed over time with a doctor.

Who MHT may suit - and who should be cautious

MHT may suit women who:

  • Have bothersome hot flashes or night sweats affecting quality of life
  • Are within a reasonable window of their menopause and generally healthy
  • Have troublesome vaginal or urinary symptoms (where local estrogen is often considered)
  • Have experienced early menopause and need support until the typical age

MHT may not be suitable, or needs extra caution, for women with certain histories - for example some hormone-sensitive cancers, unexplained vaginal bleeding, a history of certain blood clots or strokes, or particular liver conditions. This is not a complete list, which is the point: suitability is determined by a doctor reviewing your full history, not by a checklist alone.
